Saccular Aortic Arch Aneurysm with Kommerell's Diverticulum

چکیده
症例は73歳の男性．胸部異常陰影で紹介となり，CTにて右側大動脈弓，左鎖骨下動脈起始異常を伴うKommerell憩室および嚢状弓部大動脈瘤を指摘された．自覚症状は認めなかったが，嚢状瘤の破裂リスクを考慮し，手術の方針とした．手術は胸骨正中切開でアプローチした．人工心肺下，open distalとして弓部大動脈を離断後，オープンステントグラフトを挿入し，4分枝管人工血管で弓部大動脈置換を行った．人工心肺を離脱後，左鎖骨下動脈のコイル塞栓を行い，エンドリークがないことを確認した．術後，右声帯麻痺による嗄声を認めたが，他に大きな合併症なく、術後30日目に自宅退院となった．
